Acknowledging that someone is dying, respectfully talking with them and their family, including them in decision-making, supporting them and their family, and developing an individualised care plan that includes enough nourishment and water are the five priorities of end of life care.

It depends on your requirements and preferences where you receive end-of-life care. In-home care, care facilities, hospices, and hospitals are all options for receiving end-of-life care. Your right to get high-quality care extends to where you choose to spend your last moments. 

Who Provides End of Life Palliative Care

Which health and social care experts will handle your end-of-life care depends on your requirements, preferences, and location. Your family and friends may participate in your end-of-life care in addition to a medical team. Those who provide end of life care include:

  • Community or district nurses
  • Social care workers
  • Specialist palliative care professionals
  • Counsellors and therapists
  • Chaplains and other spiritual care professionals
  • Care home staff

When Should I Get End of Life Palliative Care

When you need it, end-of-life care can begin. It may last a few weeks, months, or perhaps even longer than a year. When a person’s death is anticipated within the next year, it is said that they are reaching the end of their life. It is not always possible to predict this, though. End-of-life care can help people in a variety of scenarios, everybody is different.
